Is it wise to take a personal loan to pay off credit card debt?

I have around 00 in credit card debt which I want to pay off and have a clean slate..is it wise to take personal loan from a bank to do so? I am a little confused on what is the best way out of this..

I am interested in consolidating my credit card debt. Would it be best to use another low/no interest credit card or a personal loan? Also, would it be best, once these cards are empty, to cut them up and/or cancel them? What would be best for my FICO score?
